The Rehabilitation Center, supported by the Wolk Foundation, is centrally located in Rochester, NY. Housed on site at St. John’s Home, our bright, spacious rehabilitation gym and life skills center is a warm and inviting place for you to achieve your rehabilitation and recovery goals.

Rehabilitation Expertise

Our experienced therapy staff is trained in the latest advances in rehabilitative medicine to support transitional care needs from orthopedic surgery recovery to cardiac and stroke rehabilitation. Board-certified, on-site internists/geriatricians, physician assistants, nurse practitioners, physical therapists, occupational therapists, licensed social workers, and speech therapists work together—and with you—to build an individualized plan that will get you safely and efficiently back home.

St. John’s staff delivers a full range of senior rehabilitation services, including:

Short-Term Rehabilitation After Surgery for Older Adults with Dementia

St. John’s recognizes that older adults with mild to moderate dementia require a different rehabilitation experience to ensure a successful return home after a hospital stay. Dementia is a major cause of physical disability, cognitive impairment, and dependency, which can result in an increased likelihood to be hospitalized after a fall. A customized approach to rehabilitation services that is specifically designed for individuals with dementia can ensure faster recovery rates and avoid higher levels of care following hospitalization.
